First Link® Manager – Full Time (Contract)
First Link® builds relationships with health care and other community agencies to facilitate early support and education of people diagnosed with dementia and their family caregivers.
The successful candidate must have:
- Client support and case management skills
- Experience in project management
- Collaborative leadership style
- Post-secondary education in a health or social services discipline
- 5 years of experience in the field of dementia care and family support
